5. טפסים
(א)נקבע בחוק או בתקנות אלה שיש להגיש בקשה או הודעה, לא יוגשו אלא בטופס שנקבע לאותו ענין.
(ב)לא נקבע טופס כאמור בתקנת משנה (א), יוגשו הבקשה או ההודעה בכתב בצורה שנקבעה להגשת מסמכים לרשם, ויצויינו בהן הפרטים שנקבעו בתקנות אלה וכן שמו של מגיש המסמך, מקום מושבו הקבוע, מענו למסירת מסמכים ואזרחותו.
