4. ראיות
לבקשה יצרף התובע ראיות הדרושות להוכחת העובדות שפורטו בטופס הבקשה ובתקנה 3, וכן כי –
(1)עד לעלייתו לישראל היה מחוסר נתינות או פליט כמשמעותו באמנה בדבר מעמדם של פליטים;
(2)עלה לישראל לפני יום כ״ב בתשרי תשי״ד (1 באוקטובר 1953) וביום כ״ט באדר ב׳ תשי״ז (1 באפריל 1957) היה ולאחר מכן נשאר אזרח ותושב ישראל;
(3)דרגת נכותו אינה פחותה מ־25%.
