33. צורת ההחלטות
(א)החלטת ועדת הקנסות בגמר הדיון תיחתם בידי כל חברי הוועדה והיא תכלול פרטים אלה:
(1)מספר התיק;
(2)מען הוועדה;
(3)שמות חברי הוועדה;
(4)שמות בעלי הדין ושמות באי כוחם ומענם;
(5)העובדות שהעלו התביעה וההגנה;
(6)הכרעת הדין ונימוקיה;
(7)גזר הדין;
(8)תאריך החלטה.
(ב)העתק ההחלטה לפי תקנה זו יימסר לבעלי הדין סמוך ככל האפשר לאחר נתינתה.
