3. בקשת נפגע לקבל מידע
בקשת נפגע לקבל מידע מן הקרן תוגש בכתב או באמצעות אתר האינטרנט של הקרן, ותיערך לפי הטופס שבתוספת, ויצוין בה מועד קרות תאונת הדרכים, מספר הרישוי של הרכב המעורב וכן הצהרת הנפגע כי המידע דרוש לו לשם מימוש זכויותיו בקשר לתאונת דרכים.
