2. הודעה אינדיבידואלית על כל מחלה מחלק א׳ או ב׳
הודעה על כל מחלה המפורטת בחלק א׳ או בחלק ב׳ של הרשימה – למעט מחלות שחפת, מין ומלריה – תימסר לכל מקרה ומקרה לחוד בטופס מס׳ 1 שנקבע בתוספת.
