1. בקשה לתעודת מקצוע או לשינוי דרגה או סוג
תיקון: ס״ח תשל״ז
בקשה לתעודת מקצוע או לשינוי דרגה או סוג תוגש למרכז ועדות המקצוע במשרד העבודה (להלן – המרכז), באמצעות לשכת שירות התעסוקה [במקור: לשכת העבודה] שבתחום שיפוטה גר המבקש; בקשה לתעודת מקצוע ובקשה לשינוי דרגה או סוג תהיה לפי הטופס שנקבע בתוספת.
