2. בקשת היתר
(א)בקשה להיתר להקים ולהחזיק תערוכת בעלי־חיים תוגש למנהל לפחות שלושים יום לפני המועד המיועד לפתיחת התערוכה (להלן – הבקשה).
(ב)מגיש הבקשה יציין בה פרטים אלה:
(1)מועד התערוכה;
(2)מקום התערוכה;
(3)סוג בעלי־החיים שיוצגו בתערוכה;
(4)שמות המקומות או המשקים מהם יובאו בעלי־החיים;
(5)פרטים נוספים שדרש המנהל.
