Section 2 of תקנות הכניסה לישראל (בקשה לאישור כניסתו לישראל של ילד באימוץ בין־ארצי), התשנ״ח–1998, הגשת בקשה.
2. הגשת בקשה
(א)בקשה תוגש בידי עמותה מוכרת למינהל האוכלוסין שבמשרד הפנים בירושלים לפי הטופס שבתוספת.
(ב)לבקשה יצורפו אישור של רשות מוסמכת של מדינת החוץ למסירת הילד לאימוץ למבקש וכן אישור של רופא מטעם העמותה המוכרת שבדק את הילד שמבקשים לאמצו (להלן – הילד), ושלפיו אין הילד חולה במחלה העלולה לסכן את הציבור.
