3. פרטי רישום בפנקס
בפנקס יצוינו הפרטים שלהלן:
(1)שם משפחה;
(2)שם פרטי;
(3)שנת לידה;
(4)מספר תעודת זהות או מספר דרכון;
(5)התואר או התעודה כמפורט בסעיף 9 לחוק;
(6)שם מוסד הלימודים שהעניק את התואר או את התעודה;
(7)תאריך סיום הלימודים;
(8)תאריך הרישום בפנקס;
(9)מספר הרישום.
