5. בקשה לרשיון לייצור טבק
תיקון: תש״ם
(א)בקשה לרשיון לייצור טבק או חידושו על פי סעיף 29 לפקודה תוגש לגובי המכס ירושלים או חיפה או לממונה האזורי מס קניה ומס ערך מוסף, תל־אביב, לא יאוחר מתום חודש דצמבר של כל שנה.
(ב)רשיון כאמור בפסקת משנה (א) יהא ערוך בטופס מס׳ 5 שבתוספת.
