11. בקשה להיוון קצבה
הרוצה להוון חלק מקצבתו יגיש לממונה בקשה בכתב ויציין בה את האחוז הקובע; המבקש רשאי לכלול את הבקשה בטופס תביעתו לגמלה או להגישה לממונה בנפרד, ובלבד שהבקשה להיוון תוגש בתוך שנתיים ממועד הפרישה משירות; אולם מי שאינו זכאי לקצבה עקב הוראות סעיף 35 לחוק, רשאי להגיש את הבקשה רק בתוך 60 ימים מיום שהחלה להשתלם לו קצבה.
