3. הצגת מסמך
בעת הגשת הבקשה יציג המבקש תעודת זהות או אישור מלשכת מרשם לפי חוק מרשם האוכלוסין, התשכ״ה–1965, שמהם אפשר ללמוד על גילו של הנער; מקבל הבקשה יאשר בגוף הבקשה כי הפרטים שבה, המתייחסים לגיל הנער, מתאימים לנאמר בתעודה או באישור האמורים.
