4ג. ערכה עם מפוח מטעמי בריאות
תיקון: תשנ״ד
(א)אדם שהוא או התלויים בו זקוקים לערכת מגן עם מפוח, מטעמי בריאות, יגיש בקשה על כך בכתב, בשני עותקים, לפי הטופס שבתוספת השמינית, לועדה של שלושה רופאים שמינה ראש הג״א ושקבע את היושב ראש שלה (להלן – ועדה רפואית).
(ב)הוראות תקנה 9(ד) עד (ז) יחולו, בשינויים המחוייבים, על סדרי הדיון בועדה הרפואית.
