2. צירוף מסמכים
לבקשה כאמור יש לצרף מסמכים מקוריים על מקום הגיוס, תקופת השירות הפעיל, תאריך תחילת השירות וסיומו; במקרים מיוחדים, רשאי נציב השירות לקבל מסמכים אחרים כראיה. אם שוכנע כי אין למבקש כל אפשרות להמציא מסמכים מקוריים.
