2. טופס ההצהרה ודרכיה
ההצהרה על רכוש תיערך לפי הנוסח שבטופס שנקבע בתוספת לתקנות שירות המדינה (מינויים) (הצהרה על רכוש), תשכ״א–1960, ותוגש על ידי העובד המצהיר למנהל לשכת נשיא המדינה או למי שהוסמך לכך על ידיו במעטפה סגורה.
