12. שינויים
מי שמשתלמת לו הענקה חייב למסור לגוף המוסמך הודעה, בציון מענו ומספר זהותו, על כל שינוי במקום מגוריו ובמספר ילדיו ועל כל שינוי אחר העלול להשפיע על הזכאות להענקה; ההודעה תימסר תוך שלושים יום מתאריך השינוי.
