3. הבקשה בכתב
הבקשה תוגש בשלושה עותקים והמבקש יפרט בה:
(1)שמות הצדדים לעסקה ומספרי הזהות;
(2)מהות העסקה;
(3)תנאי העסקה;
(4)התמורה הניתנת על ידי כל צד לעסקה;
(5)הנימוקים למתן הסכמת הרשות לעסקה.
