5. פרטי כתב הערעור
כתב הערעור יוגש בחמישה העתקים, וייחתם ביד המערער או בא כוחו ויפורטו בו:
(1)שמו ומענו של המערער;
(2)שמו ומענו של המשיב;
(3)התאריך שבו הגיעה החלטת הועדה למערער;
(4)המספר הסידורי של תיק הועדה;
(5)הנימוקים אשר עליהם מבסס המערער את הערעור, כל נימוק בפסקה מיוחדת ובקיצור;
(6)הסעד המבוקש.
