1. בקשה לביצוע פסק־דין
בלשכה שהופעלה בה מערכת מיכון (להלן – הלשכה) תוגש בקשה לביצוע פסק־דין לפי טופס 851 כמפורט בצו ההוצאה לפועל (שימוש בטפסים נוספים בלשכה ממוכנת). תשל״ו–1978 (להלן – צו הטפסים הנוספים), ויצורפו אליה המסמכים כנדרש בתקנה 11 לתקנות.
