1. התחייבות לביצוע תשלומים
מבקש היתר שלא קיבל מאת רשות הרישוי פירוט אגרות, היטלים ותשלומים כאמור בסעיף 158לא לחוק, ולא שילם כאמור לפי חשבון שערך בעצמו, יתן התחייבות לשלמם תוך שלושה חדשים בצירוף ערבות בנקאית, להבטחת מילוי ההתחייבות, כאמור בתקנה 2; ההתחייבות תינתן לפי הטופס שבתוספת.
