5. תעודה על ניכוי מס
כל חברה שניכתה מס מדיבידנדה תמציא למקבל הדיבידנדה, בשעת תשלומה, תעודה בה תפרט את סכום הדיבידנדה, ההכנסה ממנה היא משתלמת ואת סכום המס שניכתה.
