5. בקשה לרישיון סוקר אסבסט
המבקש לקבל רישיון סוקר אסבסט לפי סעיף 21 לחוק, יגיש למנהל בקשה לפי טופס 8, ממולא לכל פרטיו, ובהם פרטי המבקש והכשרתו, שצורפו אליו תעודות ומסמכים הנדרשים לאימות הפרטים שבבקשה וכן תצהיר המבקש.
