4. אופן הגשת מסמכי בקשת הרישיון
כל מסמך או הצהרה שמגיש המבקש לפי כללים אלה, ייחתמו בידי המורשים לחתום בשם התאגיד, ויצוין בהם תאריך החתימה; בצד כל חתימה יצוין שם החותם ותפקידו במבקש.
