4. בקשה לרישיון מטיסן
מבקש רישיון מטיסן יגיש למנהל בקשה לרישיון מטיסן באופן מקוון; בקשה כאמור תכלול את שמו המלא של המבקש ומספר הזהות שלו, ואם אין לו מספר זהות – מספר הדרכון שלו והמדינה שבה הוצא הדרכון, ואת כתובת הדואר האלקטרוני שתשמש אותו לקבלת מידע מהמנהל לעניין זה.
