2. בקשה להרשאה אישית
בקשה של עמית רופא להרשאה אישית לצורך ביצוע פעולות רפואיות בענף מומחיות מסוים, תוגש לרופא מומחה שמומחיותו בענף מושא הבקשה והוא עוסק בה בפועל, הפועל במסגרת המוסד הרפואי שבו פועל העמית רופא מגיש הבקשה; הבקשה תוגש באמצעות המערכת המקוונת, תכלול את הפרטים שבתוספת הראשונה, ואולם עמית רופא יחתום על ההצהרה הערוכה בנוסח שבחלק ב׳ לתוספת האמורה במועד המפורט בתקנה 5(ב).
