5. חובה להודיע על שינוי
בעל תעודה יודיע על כל שינוי שחל בכל פרט מן הפרטים הרשומים בתעודה וימציא למרכז הכשרה והדרכה מוכר שהנפיק לו את התעודה, לפי דרישתו, את התעודה לשם שינוי הרישום בה, באמצעות הנפקת תעודה מעודכנת, או ביטולה.
