3. כתב ערר
הערר יוגש בשלושה העתקים, ייחתם בידי העורר או בא כוחו ויפורטו בו:
(1)שמו של העורר, משלח ידו ומענו;
(2)המועד שבו נמסר לעורר העתק ההחלטה או המועד שבו נודע לו עליה;
(3)העובדות שעליהן מסתמך העורר;
(4)הנימוקים שעליהם מבסס העורר את עררו, לרבות נימוקים משפטיים, כל נימוק בפסקה מיוחדת;
(5)הסעד המבוקש.
