תובע המבקש לאשר תובענה כייצוגית יצרף לתובענה שהוא מגיש בקשה על כך, בהתאם לפרק כ׳ לתקנות סדר הדין, ויציין בה את הפרטים שבתקנה 3 (להלן – הבקשה).
