הומצאה לפקיד סעד בקשה לפי תקנה 286, ימסור פקיד הסעד לבית המשפט תסקיר הכולל ידיעות מלאות ככל האפשר לענין הפרטים הנקובים בטופס 33, ורשאי פקיד הסעד לכלול בו גם פרטים נוספים אם נראה לו שיש בהם ענין לאימוץ.
