3. פרטי היתר יצוא
היתר יצוא יכלול –
(1)פרטים מלאים על הסם וכמותו;
(2)השם והמען של המייצא;
(3)השם והמען של האדם שאליו יישלח הסם;
(4)שם הארץ שאליה ייוצא הסם;
(5)פרק הזמן שבו יבוצע היצוא.
