7. צורת רשיון ותכנו
תיקון: תש״ם
(א)רשיון לפי תקנה 6 ישא מספר, תאריך, חותמת משרד הבריאות וחתימת המנהל.
(ב)ברשיון לייצור סם מסוכן, להחזקתו או לשימוש בו יצויינו –
(1)הפרטים הקבועים בתקנה 5(1) עד (5) לפי הענין;
(2)הגבלות או תנאים, באם נקבעו על־ידי המנהל;
(3)תקופת תקפו של הרשיון;
(ג)ברשיון לרכישת סם מסוכן יצויינו הפרטים הקבועים בתקנה 5(1), (2) ו־(4).
