3. הגשת בקשה
תיקון: תש״ל־3
(א)כל בקשה לפי תקנות אלה תוגש בכתב לרשות המוסמכת לתת את התעודה או לרשות אחרת שנקבעה (בפרק זה – הרשות), ואם נקבע לבקשה מסויימת טופס לפי תקנות אלה, תוגש הבקשה לפי הטופס כאמור; היתה הבקשה לחידוש התעודה, תצורף אליה התעודה המוחלפת.
(ב)המבקש ימציא לרשות בבקשתו פרטים, מסמכים, תצלומים, תכניות, הצהרות ופרטים נוספים, כפי שתדרוש, וישיב נכונה על כל שאלה שיישאל בקשר לבקשתו.
