תושב בגיר המסכים להכללת תמונות טביעות האצבעות ונתוני הזיהוי הביומטרי שהופקו מהן, במאגר הביומטרי, יאשר בכתב את הסכמתו לפי טופס הסכמה הערוך לפי הנוסח שבטופס 1 לתוספת השנייה.
