6. הגשת תביעה
(א)תביעה לפיצוי חד־פעמי, לקיצבה או למענק לפי חוק זה תוגש למשרד הבריאות.
(ב)השר יקבע בתקנות הוראות, כללים, תנאים ומועדים להגשת תביעה לפי חוק זה.
