תיקון: תשמ״ח, תשס״ד, תשע״ז־3
חל שינוי מהותי בפרט מהפרטים שמסרה המבקשת לרשות במסגרת בקשתה או במסמכים שצורפו לה, תדווח על כך לרשות, מיד משנודע לה על כך ולא יאוחר מתום יום העסקים הראשון לאחר מכן, ותצרף את המסמכים הנוגעים לעניין.
