רישום השמות ייעשה בתעודת הזהות, לפי תקנה 3 לתקנות מרשם האוכלוסין (רישומים בתעודת זהות), תש״ל–1970, לגבי שינוי שם לפי סעיפים 6, 7, 10 עד 14 לחוק, ופקיד הרישום יסגור את השם הקודם בסוגריים.
