3. הגשת תלונה לנציב
(א)תלונה תוגש בכתב, בשני עותקים, לפי הטופס שבתוספת, כשהיא מודפסת או בכתב יד אם אין המתלונן יכול להגישה בדפוס, ותישלח לנציב במעטפה סגורה.
(ב)התלונה תיחתם ביד המתלונן ואם הוגשה בשמו – ביד מי שהוסמך להגיש את התלונה בשם המתלונן (להלן – מגיש התלונה), ותצורף לה הסמכתו החתומה.
