בקשת צד תהא ערוכה לפי טופס 1 שבתוספת, תחולק לסעיפים מסומנים במספרים סידוריים, ותכיל בצורה תמציתית את העובדות הצריכות לענין, את עיקר טענות המבקש ואת מבוקשו.
