37א. בקשה לקבל אישור
תיקון: תשנ״ב, תשס״ט
(א)בקשה לקבל אישור של בעל תפקיד לפי סעיף 5(ב) לחוק (בפרק זה – אישור), תוגש לועדת האישורים על גבי טופס 10א שבתוספת הראשונה.
(ב)בקשה לחדש אישור תוגש לועדת האישורים על גבי טופס 10ב שבתוספת הראשונה, לא יאוחר מיום 1 בדצמבר של כל שנה.
