3. הודעה על שינויים
המקבל תגמול נוסף יודיע בכתב לרשות המוסמכת על כל שינוי שחל בהכנסותיו, וכן על כל פרט אחר המשפיע על זכאותו לתגמול נוסף או על שיעורו, תוך שלושים יום מיום השינוי כאמור.
