11. פרטי הבקשה למידע להיתר
בקשה לקבלת מידע להיתר תיערך בידי עורך בקשה ותכלול נתונים אלה:
(1)פרטי המקרקעין שלגביהם מבוקש המידע להיתר, לרבות כתובתם ופרטי זיהוים;
(2)פרטי עורך הבקשה למידע להיתר, לרבות כתובת דואר אלקטרוני לצורך קבלת הודעות;
(3)פרטי מבקש המידע להיתר, לרבות כתובת דואר אלקטרוני לצורך קבלת הודעות;
(4)פירוט השימוש המבוקש ופרטי העבודה או הבנייה המבוקשת שלגביהם נדרש המידע להיתר.
