13. דרך הגשת תלונה
(א)תלונה תוגש לנציב בכתב, תיחתם בידי המתלונן ויצוינו בה שם המתלונן, מספר הזהות שלו ומענו ופרטי הנילון כאמור בסעיף 12(א) או (ד), לפי העניין; המתלונן יצרף לתלונה את כל המסמכים שבידו הקשורים לתלונה.
(ב)בתלונתו, יציין המתלונן אם פנה לערכאות או לגוף אחר המוסמך לברר תלונות לפי הדין, ויצרף את כל המסמכים הקשורים לאותה פנייה.
