12. הודעה על כניסה למוסד
(א)תוך חמישה ימים מיום שהתחיל תלמיד ללמוד במוסד יודיע מנהל המוסד על כך בכתב לרשות החינוך המקומית שבתחום שיפוטה גר התלמיד.
(ב)ההודעה תפרט את שם התלמיד, מספרו בספר התושבים המתנהל לפי פקודת מרשם התושבים, תש״ט–1949, מענו והכיתה בה הוא לומד.
