3. הודעה
(א)חתמו הורים על הצעת השלמה, יצרפו אליה הודעה בכתב וימסרוה למנהל אותו מוסד. ההודעה תהא חתומה על ידי כל ההורים הדורשים את תכנית ההשלמה ותישא את התאריך בו חתם ההורה האחרון.
(ב)ההודעה תכלול את הסכמת כל ההורים החתומים כאמור להצעת ההשלמה ואת רצונם כי הצעת ההשלמה תבוא במקום תכנית ההשלמה שנקבעה לאותו מוסד.
