הורים של תלמידים בכל כיתה של מוסד, שרצו בתכנית נוספת כאמור בסעיף 8 לחוק החינוך והסכימו על תכנית נוספת מסויימת (להלן – הצעת תכנית נוספת), יעלו את פרטיה בכתוב וימסרוה למנהל אותו מוסד בצירוף הודעה בכתב.
