2. בקשת רשיון
(א)בקשת רשיון לייצור מלט תוגש למנהל בצורה ובאופן שיורה ויש לציין בה כל פרט שידרוש המנהל.
(ב)היה המבקש יחיד תיחתם הבקשה על ידיו, ואם היה תאגיד – על ידי מי שמוסמך לחתום בשם התאגיד בצירוף חותמת התאגיד.
(ג)חלו שינויים בפרטים שנמסרו על פי תקנה זו, יש להודיע עליהם למנהל מיד עם השינוי.
