12. צירוף מסמכים לבקשה לצו לפתיחת הליכים
המבקש צו לפתיחת הליכים יצרף לבקשתו תצהיר המאמת את העובדות שעליהן נסמכת בקשתו; השר רשאי לקבוע מסמכים נוספים שעל המבקש לצרף לבקשתו לשם מילוי התנאים להגשתה.
