Section 4 of תקנות הסכמים לנשיאת עוברים (אישור הסכם ומעמד היילוד) (רישום בפנקס), התשנ״ח–1998, פנקסי עזר.
4. פנקסי עזר
(א)הרשם ינהל שלושה פנקסי עזר בצורת אלפונים לפי שמות המשפחה, שבהם יירשמו:
(1)בפנקס האחד – שם המשפחה והשם הפרטי של הילד כפי שנקבע בצו ההורות או בצו וכן שמו הקודם, אם ניתן;
(2)בפנקס השני – שם המשפחה והשם הפרטי של כל אחד מההורים המיועדים, וכן שמות קודמים שלהם;
(3)בפנקס השלישי – שם המשפחה והשם הפרטי של האם הנושאת, וכן שם קודם שלה.
(ב)ליד כל רישום תצוין הפניה למספר הסידורי של הגיליון בפנקס שבו רשום השם.
