תיקון: תשנ״א
בעד מתן היתר כאמור בתקנות 3 עד 5 ו־8 מאת הממונה על קרינה רפואית, תשולם אגרה בסכום כמפורט בסעיף 4 בתוספת השניה לתקנות אגרות בריאות, התשמ״ט–1989.
