125. בקשה לצו עיכוב יציאה מן הארץ
בבקשה לצו עיכוב יציאה מן הארץ יציין המבקש גם את שמו ואת שם משפחתו של המשיב באותיות לטיניות, וכן מענו, מספר הזהות שלו ומספר הדרכון, ואם אלה אינם ידועים למבקש – פרטים אחרים שיש בהם, לדעת בית המשפט, כדי לזהותו; בית המשפט רשאי לדרוש פרטים נוספים כדי למנוע טעות בזיהוי.
